Weeks Leading up to School-Located Influenza Vaccination (SLIV) Clinic
- Immunization Committee
- Determine who will collate consent forms by grade or teacher/homeroom teacher in alphabetical order
- Review consent forms for eligibility of vaccination (LAIV and TIV) to determine how many doses of the vaccines should be allocated per school
- Send home ineligibility letter, if applicable
- Prepare a letter to go home in response to incomplete consent forms
- Plan for a “last chance” notice to go home with students who have not returned consent forms
- Schedule volunteers to staff each clinic (clinical and nonclinical)
- Invite media to cover SLIV clinic
- Develop an “all clear” list for all consented medically eligible children
- Day of
- Create name tag for each student, provide to schools and ask teacher to put on students
- Arrive 30 minutes prior to clinic
- Keep clinics on schedule
- Account for vaccine use and removal of waste
- After
- Develop report for input into state registry
- Appropriate staff enters information into the vaccine registry
- 2 weeks from the date of the SLIV clinic
- Screen forms and contact parents if necessary
- Day of the SLIV clinic
- As doses are administered, update consent forms with the appropriate vaccine information, including lot numbers and date of administration
- Place consent forms back into school-specific binder
- Place consent forms of students in need of second dose into a separate section of the binder
- Return binders to the public health department after the clinic is complete
- After the SLIV clinic
- After entering vaccination information into the registry, hand off binders to clerical/billing department for claims to be filed with appropriate payers
- Ensure billing department has all codes needed for vaccine and administration reimbursement
- These binders will contain sensitive individual personal and health information and are therefore subject to applicable privacy laws and regulations
- Train nurses on storage, handling, and administration
- Ensure vaccine is in stock and ready to deliver on the day of the SLIV clinic
- Visit school sites to map out vaccination areas
- Take nurses through a practice session to note and address any potential challenges
